  • Project Description  (2500 characters)

  • ​Benefit Statement - How will your project benefit residents of Hyde Park / Kenwood?  Be specific about populations and how the benefits will be measured.   (2500 characters max)
     

  • Project Budget / Funder List - (current and projected grants with amounts)  (for upload - use this template)

  • 501c3 IRS Determination Letter    (upload your pdf)

  • Most recent Audit or 990 Tax Return   (upload your pdf)
